Infertility is defined as the inability to conceive after one year of unprotected sexual intercourse or six months in women over 35. It can be categorized into primary infertility (no prior pregnancies) and secondary infertility (at least one previous pregnancy).
Approximately half of infertility cases are attributed to female factors, 20-30% to male factors, and 20-30% to both genders. Sometimes, specific causes remain unidentified. General health and age typically play significant roles in fertility, particularly for women.
For women, fertility years begin with menstruation and egg release, gradually declining 5-10 years before menopause. A decline in women’s fertility awareness starts around age 32, accelerating after 37 due to diminished ovarian reserve. Male fertility also decreases with age, especially after 40-45 and notably beyond 50.
Ovulation is the release of an egg from a woman’s ovaries, essential for pregnancy. It occurs approximately 36-40 hours after a surge in a hormone called luteinizing hormone (LH). Once released, the egg can be fertilized by sperm along its path through the fallopian tube.
Issues with ovulation or its absence are major contributors to infertility. Knowing when a woman is on the verge of ovulation can be particularly helpful for couples trying to conceive, aiding in strategic timing during sexual activity.
Regular menstrual cycles between 21 and 35 days are indicative of ovulation. Various methods exist for ovulation testing, including basal body temperature (BBT) charting, urine test kits measuring LH levels, blood tests for specific hormones, and transvaginal ultrasound.
Infertility evaluation involves examinations and tests to identify the cause of you and your partner’s inability to conceive. If a cause is found, treatment may be possible. In many cases, infertility is successfully treatable even if a specific cause isn’t identified.
If you have not conceived after 1 year of regular unprotected sexual intercourse or 6 months for those over 35, specialists recommend infertility evaluation. If you are over 35, an evaluation is advised after 6 months of trying. If you are over 40, consult with an ob-gyn now for assessment and potential treatment.ر
